When you schedule your first appointment, you will receive an e-mail with paperwork to complete online. You can also access the forms below. These forms are needed in advance of your dental treatment. Please either complete the forms online or arrive 15 minutes early to your appointment to complete them in the office. You can expect your initial appointment to last approximately one and a half hours. Oral hygiene instructions will be provided along with suggestions to help you care for your teeth. In most cases, we will also clean your teeth at this visit and provide an evaluation that will outline any existing dental problems and proposed treatment.
