If you are one of our many patients without dental insurance, our plan might be just what you’ve been looking for! This plan is only for existing patients without dental insurance coverage and provides a savings of $50 a year for your preventative services.
Membership Plan Details:
- No deductible

- Open enrollment December 1 - May 31
Adult Smart Smile Club Membership
Benefits:
- 2 Cleanings
- 2 Periodic Exams
- 1 set of Bitewing x-rays
Membership Fees:
- Cash or Check: $375
- Credit or Debit: $395
Child Smart Smile Club Membership
Benefits:
- 2 Cleanings
- 2 Periodic Exams
- 1 set of Bitewing x-rays
- 1 Fluoride Treatment
Membership Fees:
- Cash or Check: $350
- Credit or Debit: $370
Terms and Conditions
- Membership is for one year beginning January 1 and ending December 31.
- Membership is available only to established patients without dental insurance.
- Membership enrollment fees must be paid in full prior to any dental care procedures.
- If the member does not utilize their plan benefits within the benefit year, the plan is NON-REFUNDABLE and offers no additional discounts.
- Member is responsible for contacting the office to schedule services in a timely manner.
- Barnes Dental Associates is not responsible for missed benefits due to scheduling conflicts.
- The plan is for individual use only. It is not a group benefits plan.
- Membership benefits are not transferable, have no cash value, and may not be redeemed for cash.
- Membership will automatically renew on January 2nd unless a written request to cancel is received 30 days prior to the renewal date.
- Member gives permission for Barnes Dental Associates to process their credit card on January 2nd unless written request to cancel is received 30 days prior to the renewal date.
- Annual membership fees may be adjusted annually.
- Member will be informed of any increase in annual membership fee prior to renewal.
- Members are responsible for notifying Barnes Dental Associates of any address or contact changes.
- Membership must be current to receive benefits.
- Membership benefits apply only to the services detailed in the plan and do not apply to any other fees, products or services.
- Barnes Dental Associates may terminate the membership by written notice to the patient when terminating the doctor-patient relationship or discontinuing the membership program.
- This is not an insurance plan and is not subject to regulation by the state department of insurance.
- Plan membership cannot be combined with current dental insurance plans.
- No insurance claim will be filed for members under this plan.
- Payments for additional dental services are the member’s responsibility.
- Barnes Dental Associates retains the right to interpret the program stipulations.
